Read More ... The diagnosis of neonatal infection is challenging because traditional markers of infection are often misleading instead of diagnosing. This study was conducted to determine Procalcitonin (PCT) level as an early marker for diagnosis of suspected sepsis in neonates in comparison with C-reactive protein, blood culture, total and differential white blood cell counts. From August, 2008 till March, 2009, 114 blood samples were collected from neonates (males and females) admitted to the pediatricsneonatal wards at Tikrit Teaching Hospital who were clinically suspected with sepsis as diagnosed by the physician in the hospital with any features suggestive of sepsis. The results of blood culture showed that 53 (46%) of samples were positive, of them 32 (60%) were males and 21 (40%) were females. The number of negative cases were 61 (54%), of them 31 (51%) were females and 30 (49%)were males. The results of identification showed that the most frequent bacterial isolates were Coagulase Negative Staphylococci and Nocardia spp. (22% for each), followed by Listeria monocytogenes 17%, Klebsiella pneumoniae, 9% Pseudomonas aeruginosa 6% and Staphylococcus aureus 6%, while (Citrobacter diversus, Serratia marscesens, Serratia liquifascens, Lactobacillus spp., Peptostreptococcus spp., Micrococcus spp., Streptococcus pyogenes) were frequently less isolated recording 2% for each . The antibiotics susceptibility test revealed that Gram negative isolates showed high resistance to antibiotics used in the present study. Each isolate of the Gram positive bacteria showed different pattern of resistance against the antibiotics used. Results of virulence factor showed that 85%, 100%, 100% , 100% and 62% of Gram negative isolates and 61%, 100% , 89%, 94% and 78% of Gram positive isolated produced Urease , Hemolysin, Lipase, Lecithinase and alkaline protease, respectively. Results of routine hematological tests at which the differences were statistically significant showed that 33% of culture positive cases had leucopenia, 19% of cases had leukocytosis, while the rest were in the category of normal range. For CRP, the differences were not significant among culture positive and culture negative cases. With reference to PCT test which had been measured using the immunochromatographic semi-quantitative method. The results showed that 21% of culture positive cases were with PCT level with high levels distributed among levels of PCT≥0.5 ≥2 ≥10 ng/ml. As for culture negative cases, 49% of PCT were systemic infections cannot be excluded though they were culture negative. In addition PCT results did not fit with CRP all the time since some cases recorded higher level of PCT ≥10 ng/ml despite they were negative for CRP. The comparison between results of PCT and WBC for the same performed cases showed that 68% of cases were in the category of normal WBC range, meanwhile they had high level of PCT. Only 18% were in the category of leukocytosis, and the rest of percentage were in the category of leucopenia. When PCT was compared with other routinely measured parameters used in this study, It seemed to be significantly more sensitive in sepsis diagnosis. Accordingly, we can conclude that PCT is a more sensitive marker than other traditional tests (blood culture, CRP and WBC count) that could be used in early diagnosis of bacterial sepsis in neonates

Read More ... The present investigation represents a preliminary study of the effect of crude extracts (aqueous, ethanolic and hexanic) of date pits (Phoenix dactylifera L. cv. Zahdi) on two malignant cell lines (human laryngeal carcinoma-Hep2 and murine mammary adenocarcinoma-AMN3) and one normal cell line (rat embryo fibroblast-REF). The study also includes evaluation of the effect of these extracts on several cytogenetic parameters such as mitotic index (MI%), blast index (BI%) and chromosomal aberrations (CA) after in vitro culture of peripheral blood lymphocytes. This work also includes a study of the therapeutic potential of one of these extracts in the treatment of transplanted murine mammary adenocarcinoma in mice.The in vitro cell growth assay showed that there was time- and concentration-dependent cytotoxic effects of crude extracts of date pits on Hep2 and AMN3 cell lines. The highest significant effect of these extracts was achieved after 72 hrs of exposure with the highest concentration (10000 μg/ml). Ethanolic extract of pits (EP) caused growth inhibition percentage 89.4% , 93.4% for Hep2 and AMN3 respectively. However, 72 hrs exposure to EP at concentration of 10000 μg/ml caused slight inhibitory effect on REF cell line, reaching 17.7%.On the other hand, the crude extracts of pits caused significant reduction in the mitotic index and blast index of peripheral human lymphocytes, but without any structural or numerical chromosomal aberrations. Also these extracts neither replaced phytohemagglutinin (PHA) as mitogenic agent, nor colcemide as mitotic arresting agent at metaphase.The therapeutic doses of EP were determined according to LD50 in mice. The results indicated high effectiveness of this extract in a dose- and time-dependent manner. The highest therapeutic doses of EP (1 gm/kg B.wt.) showed the best therapeutic effect by reducing the tumor volume in mice to about 83.8%.The comparison of relative tumor volumes of different groups revealed highly significant differences among all treated groups and those of untreated (control) group.

Pneumocystis jirovecii is an opportunistic eukaryotic pathogen causing life threating pneumonia ( PJP) in immunosuppressed patients.Detection of Pneumocystis jirovecii Pneumonia in immune suppressed patients like hematological cancer with or without chemotherapy, patients on corticosteroid therapy, other malignancy like bronchogenic cancer by using External Polymerase Chain Reaction (External PCR) and to compare the results with other control group (patients who are immunocompetent suffer from pneumonia).A total of 220 specimens were obtained from patients admitted to Al- Ramadi Teaching Hospital, Clinic excitability of respiratory disease in Al- Ramadi and Oncology Teaching Hospital in Baghdad during the period from February to July 2013. This number composed of 71(32%) specimens were from female while the male specimens were 149(68%). A total of 184 (84%) were immunocompromised patients, They were considered immunocompromised patients because they have ( hematological cancer with or without chemotherapy, patients on corticosteroid therapy, other malignancies like bronchogenic cancer). The other 36 (16%) patients with pneumonia were included as immunocompetent patients (No hematological cancer, No chemotherapy, No immunosuppressive drug like corticosteroid therapy and No malignancy like bronchogenic cancer). A total of 220 specimens which were collected, (consisting of 115 blood, 65 sputum, 40 BAL Specimens )the total 220 specimens (115 blood, 65 sputum and 40 BAL Specimens ) were examined PCR technique ( External and Nested PCR ) to identify Pneumocystis Jirovecii DNA based on the amplification of specific primers. The result of agarose gel electrophoresis revealed that small DNA diagnostic bands were detected at 346 bp for External PCR in all positive specimens. Out of 220 specimens 26 (11.8%) were detected by the mtLSU rRNA External PCR. Regarding the use of External PCR technique for sputum specimens 8 (12.3%) were positive, While BAL 8 (20%) and Blood 10 (8.7%) . PCR is more accurate and better indicative for the presence of Pneumocystis jirovecii Pneumonia. In addition this technique can use specimens which were not stored properly and became non suitable for microscopy.

The current study included understanding the effect of methotrexate and omega-3 in the thyroid and parathyroid gland and some of the physiological and histological and genetic variables in some of the organs of the body in the male rats. The study included 56 animals were divided into three groups, the first and second groups were divided into three secondary groups, the first secondary group was given the drug methotrexate only in concentration 0.05 , 0.125 and 0.250 mg / kg for 8 weeks while the secondary second group was given drug methotrexate in the same concentrations of the first group, as well as give it omega-3 concentration of 300 mg / kg for 8 weeks, the third group was considered as control group. After finishing of dosage, the animals killed and took blood samples and organs to do the required tests, included measuring of blood parameters, such as RBC ,PCV ,Hb ,MCV, MCH and MCHC. In addition to measure the number of white blood cell count WBC and Differential white blood cells, where the results showed the presence of a rise in both the RBC, PCV, Hb in the low and medium concentration of methotrexate while decreased in high concentration, omega-3 work to reduce the variables above in the low and medium concentration while worked on reducing it in the high concentration, moreover a variable of MCV and MCH and MCHC has not changed morally. With respect to the WBC was low in the totals for the study when giving methotrexate while rose when giving omega-3 with this druge, also neutrophil cells was low and accompanied by a rise in a monoytes in all concentrations of Methotrexate were not for omega 3 noticeable effect on these cells decreased. The results showed a decline in the lymph and acidophilus cells in the low concentration while rose in the medium and high concentration of Methotrexate. Omega-3 worked on the rise it in concentrations above, with respect to basophiles cells increased in the low and high concentration and decreased in medium and the omega-3 was not effect of them.
